Responsibilities

The Caseworker will work to improved functioning during home visits and coach families to better meet their child’s emotional/behavioral needs to prepare them for when they return home. The caseworker will assist facilitating the intake process and complete required paperwork. The caseworker will work closely with the Program Coordinator on individualizing the service plans for the children in the program. The caseworker will interface with school personnel, therapist and other community providers. The caseworker will work with families to create individuals learning plans based on their child and family’s needs. The caseworker will help the program maintain Behavior Support Plans for each youth in collaboration with parents, school, clinicians, and other relevant community providers. The caseworker will work with the program coordinator to facilitate SSI and Medicaid applications as needed for the children in the program. The caseworker will maintain a timeline of paperwork and authorizations in accordance with Medicaid billing standards. The caseworker will transport youth to and from home visits and provide supervision and support during home visits as needed. The caseworker will support that needs of the program as directed by the program coordinator. 

Qualifications

Bachelor’s degree in psychology, social work, or related field and 1-3 years experience with similar populations. A valid NYS driver’s license with registered and insured vehicle; Must be able to use personal vehicle for work purposes; NYS Central Registry Clearance, NYS Justice Center Fingerprinting and background check. Customer service, interpersonal, creative problem solving, communication and negotiation skills, computer proficiency.
