Responsibilities

The Care Manager will provide comprehensive care management, care coordination, comprehensive transitional care, youth & family support & health promotion. Provide referrals to community & social support services. Provide comprehensive assessment and reassessment of youth as needed inclusive of medical, behavioral, rehabilitation, social service and long term care needs. Prepare crisis intervention plan and participate in its execution as needed Ensure the delivery of all aspects of care coordination including work with service providers and health plans to provide necessary care for youth.

Qualifications

A Bachelor’s of Science or Arts degree or NYS licensure and current registration as a Registered Nurse with two years relevant experience; or a Master’s Degree with one year relevant experience required.  Demonstrated ability to work with people of diverse backgrounds required. Degrees in Health or Human Services field preferred; experience working with an electronic health record system preferred. Experience working with children and families preferred. Thorough working knowledge of local and regional public and private resources needed.
